RN/LPN MDS Coordinator

This is a PT position and has the potential to transition into a full time position. Wound Care Nurse
WOUND CARE NURSE Carriage Hill Health and Rehab, Center is currently looking for Wound Care Nurse.
Qualifications:
Licensed in good standing as a nurse with Virginia Department of Health Demonstrate clinical skills necessary to provide wound care and preventative measures Certification as a wound care nurse Demonstrates excellent customer service and leadership skills
Essential Duties:
You must have the nursing ability to complete a full body assessment on all residents
Review Care Tracker reports
Measures all wounds weekly
Initiates and performs wound care following facility protocols and physician orders
Communicate with the physicians on updates and any new orders received
Documentation on care plans are updated timely to accurately reflect the resident’s condition and needs
Have good communication skills with the nursing team
LPN and CNA
Carriage Hill Health and Rehab Center is currently looking for part time Nurses and CNA’s , Floor positions for team nursing on our new transitional unit. Must be motivated, LTC experience preferred
