Hi, my name is Colleen. I began working at Carriage Hill in April 2014 as their Referral Development Coordinator and was promoted to Director of Admissions in May 2015. My objective is to help you, or your loved one, transition from a post acute hospital stay to our Short Term-Rehabilitation Program. For many, this time of transition can be full of uncertainty, fear and distress for both patients and their families. I am here to help “de-stress” this process. You will see me in the hospital meeting potential rehabilitation patients and families for convenient on-site assessments and consultations where we will discuss goals of care, what to look for in a facility when making the decision on what is right for you, as well as what to expect at Carriage Hill. I will also work closely with your hospital Case Manager throughout the transition process to coordinate securing the information and paperwork needed for admission to our facility. Having worked as an Admissions Director for 5 years prior to working for Carriage Hill, I look forward to the opportunity to meet you or your loved one and to help her “get well and back home.”
Colleen joined our center in April 2014. She has five years experience in long term care.
Colleen graduated with a Bachelors in Psychology from the University of Mary Washington. Prior to joining CCR she worked as an Admission Director at Westmoreland Rehab.
Colleen grew up in Frederick, Maryland and Colonial Beach. She is currently a member of the Chamber of Commerce – Fredericksburg.
