The CCR Shining Star Program is an initiative to bring to light the fantastic members of our Carriage Hill Family. To nominate an individual for their stellar service, please fill out one of our nomination forms in the lobby and include:
1. The name of the individual and their unit or department
2. The act of caring or exceptional service
3. Your signature and the date
