Dear Friends of Carriage Hill,

As the COVID-19 pandemic evolves, we continue to review and implement the most current guidance from federal and state agencies as we return to a state of normalcy.

Because we are in a relatively higher-risk environment, our approach to resident and patient visitation is in their best interest for safety. Below are key visitation points I would like to draw to your attention:

  • Scheduling is on a first-come, first-served basis by utilizing our online scheduling portal for both indoor and outdoor visits. Here is a link to the portal: https://carriagehill-rehab.com/patient-visitations/
  • Whereas window visits are not scheduled via the portal, we ask that you call ahead and advise us of the estimated time for the window visit to avoid conflicts with patient care and for privacy.
  • Visits may be scheduled for 45 minutes in duration, 7 days per week, between the hours of 10:00 am and 5:00 pm for both indoor and outdoor visits.
  • Each patient is allowed 3 visitors per indoor visit and 4-5 visitors for outdoor visits. Visitors must be age 12 or older.
  • Upon your arrival, all visitors must enter through the Main Entrance to be screened in the Main Lobby. Our Concierge will direct you to your visitation venue.
  • If the patient is in a private room and both the patient and visitor(s) have been fully vaccinated, the visit may occur in the patient’s room.
  • If the patient and all visitors have been vaccinated and the visit is in a private area, visitors and patients may remove their masks during the visit. All visitors, regardless of vaccination status, are required to wear a mask in all other areas of the Center.
  • For patients and visitors who have not been vaccinated, we are recommending outdoor visits.
  • Newly-admitted, unvaccinated patients in their 10-day quarantine may not have an indoor visit until their quarantine period is satisfied; however, a window or virtual visit will gladly be accommodated.

Please know that we are diligently working to protect the well-being of our patients. Thank you for being a part of the Carriage Hill Health & Rehab Center family.

Sincerely,
